Is Asunción safe for women?

Asunción is like most cities. You have to be careful with your stuff. Most people use their phones on the street in the busy neighborhoods, but I wouldn't recommend it in evening hours when it is already dark. Pickpockets are quite common on public transport, especially when it's crowded. Like any other city, you should avoid going to the known unsafe neighborhood.
